Maternity insurance is a type of health insurance that covers eligible pregnancy and childbirth-related medical expenses, subject to the policy’s terms, conditions, limits and waiting periods. For couples planning a family, maternity coverage can help manage the potentially high costs associated with pregnancy and delivery.

However, understanding the maternity insurance claim process is as important as choosing the right policy. Knowing the steps involved and keeping the required documents ready can help make the claim process more organised.

Steps to file a maternity insurance claim

Health insurance policies with maternity coverage may allow eligible policyholders to file either a cashless or reimbursement claim, depending on the policy terms and treatment facility.

Cashless maternity insurance claim

A cashless claim generally involves the insurer settling eligible expenses directly with a network hospital, subject to policy terms and approval.

Step 1: Inform the insurer
Notify the insurance provider or the designated claims team about the planned hospitalisation within the timeframe specified in the policy.

Step 2: Submit the required documents
Provide the required documents at the hospital’s insurance or help desk. These may include the health insurance card, policy details, identification documents and pre-authorisation form.

Step 3: Hospital submits the request
The hospital generally sends the pre-authorisation request and supporting documents to the insurer or its Third-Party Administrator (TPA).

Step 4: Insurer reviews the request
The insurer reviews the submitted information to determine whether the proposed treatment is covered under the policy and whether the cashless request can be approved.

Step 5: Treatment is undertaken
Once cashless approval is granted, the hospital can proceed with the covered treatment, subject to the approved amount and applicable policy conditions.

Step 6: Insurer settles eligible expenses
The insurer settles the eligible hospital expenses directly with the network hospital. Any non-covered expenses, deductibles, co-payments or amounts exceeding applicable limits may have to be paid by the policyholder.

Reimbursement maternity insurance claim

Under a reimbursement claim, the policyholder generally pays the eligible medical expenses initially and subsequently seeks reimbursement from the insurer.

Step 1: Notify the insurer
Inform the insurer about the hospitalisation within the timeframe specified in the policy. Delayed notification may affect the claim process, depending on the applicable terms.

Step 2: Undergo treatment
Receive treatment at an eligible hospital and pay the applicable medical expenses as required.

Step 3: Collect the documents
Keep the original bills, payment receipts, prescriptions, diagnostic reports, discharge summary and other relevant medical records.

Step 4: Submit the claim
Complete and sign the maternity insurance claim form and submit it along with the required supporting documents within the stipulated timeframe.

Step 5: Claim assessment
The insurer reviews the claim form and supporting documents to determine coverage and the amount payable under the policy.

Step 6: Receive the approved amount
If the claim is approved, the eligible reimbursement amount is generally transferred to the policyholder’s registered bank account, subject to the insurer’s claim process and policy terms.

Documents required for a maternity insurance claim

The documents required may differ between insurers and policies. Commonly requested documents include:

  • Health insurance policy document or insurance card
  • Duly completed and signed claim form
  • Doctor’s prescription and consultation records
  • Diagnostic reports, including ultrasound scans and laboratory test results
  • Hospital admission advice or recommendation from the treating doctor, where applicable
  • Original hospital bills and payment receipts
  • Itemised medical bills
  • Hospital discharge summary
  • Pharmacy bills and prescriptions for medicines
  • Identity proof of the insured person
  • Bank account details and cancelled cheque, where required
  • Any additional documents or information requested by the insurer

Keeping these documents organised can help avoid unnecessary delays during claim processing.
