Revenue officials issue death certificate instead of birth certificate for infant in Khammam

In Khammam district, a clerical blunder by revenue officials at the Kusumanchi Tahsildar’s office resulted in a death certificate being issued instead of a birth certificate for an infant girl.

Khammam: In a bizarre case of official negligence, revenue staff at Kusumanchi tahsildar office in Khammam district issued a death certificate instead of a birth certificate for a girl child.

Kadari Mada Vidhya, daughter of Upender and Mamata from Gattusingaram village in Kusumanchi mandal, was born on November 12, 2022. The parents initially approached the gram panchayat office for her birth certificate, but officials directed them to the Kusumanchi tahsildar office, citing a lack of birth records.

The couple applied at the tahsildar office on December 17, 2024, but officials delayed issuance, stating that the proof of birth documents were not in proper order. When Mamata visited the office on August 4, the officials finally issued a certificate — only for her to realise it was a death certificate instead of a birth certificate.

Shocked, Mamata confronted the section officer, who tore up the incorrect certificate and handed her the correct one. But the ordeal did not end there.

When she approached the gram panchayat office to upload her daughter’s birth details online, the secretary refused, pointing out that the birth hospital was not mentioned in the certificate. On returning to the tahsildar office, Mamata faced rude behaviour from the same section officer.

She then brought the issue to the notice of tahsildar Ravi Kumar, who asked her to return on Thursday and finally issued a proper birth certificate.

Mamata expressed deep concern over the indifferent attitude and negligence of the revenue officials throughout the process.
